Position Overview
This position is responsible for the direct care of clients who require personal care services including compliance with policies and procedures, addressing individual client needs, transportation assistance, medication support, and providing personal caregiving services.
Key Responsibilities
- Assist with client admission and discharge process
- Transport clients to appointments and activities
- Assist clients with daily living skills and wellness activities
- Conduct community meetings and daily groups
- Document daily progress notes for assigned clients
Requirements
- High School Diploma or GED Certificate
- At least 21 years of age
- Current Certification in Personal Caregiving Services
- CPR and First Aid Certification
- Valid Driver's License and Fingerprint Clearance Card

Locations
We have locations throughout the Phoenix West Valley, providing quality care in comfortable, home-like environments.
All applicants:
- Must be at least 18 years of age
- Must have a high school diploma or GED
- Must be 21 to become a driver with a qualifying Motor Vehicle record provided
- Must be a caring individual
- Must be able to pass a background check to obtain a Level 1 Fingerprint clearance card from the Arizona Department of Public Safety
- Must be able to read, write and speak fluent English
- Must be able to work in a team environment
- Training preferred: Prevention & Support/CPR, First Aid, Article 9, Valid Caregiving/CNA Certificate
- Prior experience a PLUS!
